India has made remarkable achievements in areas like Polio elimination, lowering fertility and disease control. However, our progress in health outcomes has been slower in comparison to other countries with comparable incomes and at similar stages of development. Impressive gains in per capita income should match with increase in life expectancy or health status. We now face a triple burden of disease. Out of pocket expenditures in India is high (70 percent of total health expenditure). This is catastrophic for the poor and pushes an estimated 37 million into poverty every year.

Health is a subject allotted to the State List, under the Seventh Schedule of the Indian Constitution. The Central Government is jointly responsible for items in the Concurrent List.We have one of the most expansive publicly provided networks of health facilities yet issues of regional disparity, access and quality remain. The private sector despite being utilized by the majority of the population also has issues of quality and cost.

Even though the Union Budget allocation for the Ministry of Health and Family Welfare in 2015-16 has remained at the level of revised expenditure in 2014-15, an opportunity lies in encouraging States to spend more on the social sector with greater devolution of untied funds following the recommendations of the Fourteenth Finance Commission.

India is brimming with possibilities. Successful conduct of election, Census survey, projects in space and atomic sciences are some examples. India is termed as the “pharmacy of the global south”, providing affordable, life saving generic medicines to developed and developing countries. In the same way, there is potential for our health system to deliver optimal outcomes to the population.

The Twelfth Plan charts the path towards strengthening health systems so as to reach the long term objective of Universal Health Coverage (UHC). It is our belief that a Health System Strengthening approach is the solution to bridging the gap between our current status and potential performance. The Health Division of the NITI Aayog invites you to an open and informed discussion to elicit ideas for overcoming the enormous challenges in the sector with limited funds at hand and guide future action at all levels, in our system. Your opinion is important and valued.

Detailed note on the current status of health system in India

We invite your responses on these two questions:

1. How can we maximize health returns from available resources?
2. How can we increase investments in health?

This discussion is open for the next two weeks after which we will post a summary of the ideas generated on the forum. We will also follow-up with a more detailed note on the issues in health system and learning from best practices in the country as well as globally for further discussion. Informed briefs on the above mentioned questions will also be made available after the initial two weeks for more a targeted dialogue.

Subject : Expensive Dialysis treatment. Dear Prime minister, I am Ajay Nair residing in Pune. I ame suffering from Kindey Failure since last 2 yrs. I am under going dialysis thrice a week in Columbia Asia hospital. My monthly expenses only for dialysis and injection comes to around Rs.30000/-pm. I dont have a job now and no company is giving me job. There is no insurance cover since i have not taken Health insurance. There are many people like me undergoing dialysis. can govt help.

I suggest Government to Launch the scheme on Health care Insurance on Social security part.It is much more needed for today's scenario where medical expense are too much expensive for a common man.They can not afford a good medical services.For This Government It's a really possible for scheme as i think.It's a biggest success of a Government and win a heart of crores of people if They launch scheme on Health care policy in low budget cost.

There is Heavy Corruption in Private medical practice.It is more than what is happening in Govt dept. Patients are requested to undergo various unnecessary tests. Patient is prescribed heavy dose of medicines to facilitate sell of medicines .Lady is operated for ceaserian though normal delivery could be possible.Dead Patients are kept on ventilator. Angioplasty/kidney removal is done for dead patients. There is unending list of malpractice in private medical practice. Something should be done.

In India there is need of Special Hospital meant for Sr.Citizen. In old age persons having multiple diseases, for which a special hospital is the need of today for Sr. Citizen. Its name should ancient like VAN PARSTH HOSPITALS. Also like Child Care Leave, there is need Parents care leave for Govt. Employees, so that they take care their old parents.

WardWise Domestic Health Care Systems Health sys in India are grossly deficient & almost non-existent for smooth consistent health care & maternity services. Govt hosps Health Centres & path-labs should all be decentralized like other developmental facilities. In urban areas health care shd be given at ward level or even subward level. May be an ordinary facility with a bare-footed doctor, but regular & closeknit in the domestic ward or subward area with regular Health Card for every Citizen.

The condition of health in rural area of India specially in Bihar is very bad.One hospital should established in one GRAM PANCHAYAT so that no one can die just beacuse not medical service.There is thousand of JHOLACHHAP doctor are running there business in rural area and making people fool.Threr is huge demand of medical facilty in rural area.Rural people are feeling ignored just because not proper medical facilty .

When a patient goes to a government hospital, they have to take their entire case history with them. Instead of this, what can be done is we can take the AADHAR card number of that particular patient and maintain a digital record for that number on an online system. In this way, we can create a nationalized database and the patient can be treated wherever in the country they may be,without the need to produce the case history.The doctor can easily download it based on the patients aadhar number.

#NITIAayog : How to increase investment ? Fiscal prudence demands more income and/or judicious spending. We have to tackle at the root of the problem. Focus at plugging loopholes in social security / execution of projects. If 50,000 crore are allocated & 20% of it reaches the people, amount reaching is only 10,000crore. Reduce allocation to 30,000 cr & if 40% reaches, amount reaching will be 12,000 cr. It is a win-win situation for govt. & people. Money saved can be used for Health & Education.

The success of this scheme will be depend up on the rate of interest on gold deposited. A good interest rate may force people to open up their lockers to deposit gold. In my opinion, a minimum 3% interest p a is necessary for the success of the scheme.
