Additional suggestions: What India needs are not necessarily more AIIMS, that is large, super advance facilities, that make health care more expensive, but smaller, modern facilities with high speed broadband that allows remote assessments of patients. Supplement this with simple, but clean housing, boarding facilities for doctors and nurses. Make it possible for them to WANT to go and work in such places, and at the same time get expert advice from doctors, specialists, not physically present

Additional suggestions: Please review all the centrally funded institutes and put in place a central mechanism outside of these institutions for the patients to rate their services. You can supplement that by getting experts in public health to review the institutions too. You are likely to find that some of the institutions do much more for the citizen than others. E.g: NIMHANS, Bengaluru serves people from all over the country at a very low cost. Learn how they do this, and aim to replicate

Indian Health Care System has become Chaotic, Untrustworthy, completely jeopardized due to serious CLASS-Division & CASTE LIKE SYSTEM prevailing in Medical Education System. In such a scenario the question of "how to maximize health returns from available resources" and "increase investments in health" becomes tricky. I have tried to analyze the problem from the MEDICAL DOCTORS Perspective who I feel is totally confused and marginalized today. Please go through the attachment for details.

1. How can we maximize health returns from available resources?
A: Tighten the auditing of expenditure accounts which is likely to ensure catching pilferage of precious resources for personal gain, by the administrative staff associated with all Central/State govt funded hospitals.
2. How can we increase investments in health?
A: Focus should be on prevention. Hence need to be able to determine root cause, and to be able to take executive actions, if needed, to remove it from the environment

Make the existing Govt. hospitals more patient friendly with more facilities.Make sure that the doctors, staff and the instruments are working properly/timely.Supply enough medicines and staff to the hospitals.Monitor the functioning of the hospitals regularly.Provide registers for putting complaints/suggestions and also online to health ministry.Give awareness programme to the public especially in the rural/remote areas.

The number of general hospitals at district and block level should be doubled keeping in mind our increasing population.The distribution of block level hospital should be located according to the geographical map of the concered area so that no area in left uncovered.

Although increasing population is a detterent, smaller distances in our country can be utilised to our benefit. High quality curative health facilities should be concentrated at district and block level instead of scattered at village level. The patients can be readily transported to such 24X 7 hours health facilities which will be shining as stars once fully operational. PHC level health facilities should cater to preventive public health services only.

Regarding shortage of manpower: A system should be formulated whereby all postgraduate students should serve in the health system for extra two years before they can be confered with the degree, at choice of their station across entire country where they can persue two year highly paid professional service in government health system.
Benefits of this formula: The health system will have specialist docotrs to serve general population. Also,myht of shortage of doctors will also be busted.

Benefits of three different cadres:
1. Funding can be increased systematically.
2. Functioning of health system will become more efficient and productive.
3. Government will have good hold on the health system
4. Accountability of the health system will increase.

Presently,a single cadre of health system is performing all health related task. Now time has arrived when our country should have three different cadres catering to different health related needs of our population.They are as follows:
1. Cadre to deal with emergency including surgical interventions and medicolegal cases.
2. Cadre to deliver curative treatment to general population.
3. Cadre to deal with preventive health services.
All these cadres should function under NeHA.
