India has made remarkable achievements in areas like Polio elimination, lowering fertility and disease control. However, our progress in health outcomes has been slower in comparison to other countries with comparable incomes and at similar stages of development. Impressive gains in per capita income should match with increase in life expectancy or health status. We now face a triple burden of disease. Out of pocket expenditures in India is high (70 percent of total health expenditure). This is catastrophic for the poor and pushes an estimated 37 million into poverty every year.

Health is a subject allotted to the State List, under the Seventh Schedule of the Indian Constitution. The Central Government is jointly responsible for items in the Concurrent List.We have one of the most expansive publicly provided networks of health facilities yet issues of regional disparity, access and quality remain. The private sector despite being utilized by the majority of the population also has issues of quality and cost.

Even though the Union Budget allocation for the Ministry of Health and Family Welfare in 2015-16 has remained at the level of revised expenditure in 2014-15, an opportunity lies in encouraging States to spend more on the social sector with greater devolution of untied funds following the recommendations of the Fourteenth Finance Commission.

India is brimming with possibilities. Successful conduct of election, Census survey, projects in space and atomic sciences are some examples. India is termed as the “pharmacy of the global south”, providing affordable, life saving generic medicines to developed and developing countries. In the same way, there is potential for our health system to deliver optimal outcomes to the population.

The Twelfth Plan charts the path towards strengthening health systems so as to reach the long term objective of Universal Health Coverage (UHC). It is our belief that a Health System Strengthening approach is the solution to bridging the gap between our current status and potential performance. The Health Division of the NITI Aayog invites you to an open and informed discussion to elicit ideas for overcoming the enormous challenges in the sector with limited funds at hand and guide future action at all levels, in our system. Your opinion is important and valued.

Detailed note on the current status of health system in India

We invite your responses on these two questions:

1. How can we maximize health returns from available resources?
2. How can we increase investments in health?

This discussion is open for the next two weeks after which we will post a summary of the ideas generated on the forum. We will also follow-up with a more detailed note on the issues in health system and learning from best practices in the country as well as globally for further discussion. Informed briefs on the above mentioned questions will also be made available after the initial two weeks for more a targeted dialogue.

In so far as the optimum utilisation of the current #HealthSysteminIndia is concerned, we will have to develop an information technology portal that is a repository of the patient data with the family doctor. So, when he suggests his patient to a say,,, cardiac surgeon, he has a complete history of that patient. Such systems do exist in India. However they are only limited to hospitals. For e.g. Even in metros today, you will not find a PC in the doctor's clinic.

Health System in India especially Public Health deteriorated due to many reasons.To improve and bridge the gap a wholistic approach must be followed i.e. combination of Allopathy, Homeopathy, Ayurvedic and other proven alternative system. British Public Health Model must be studied rather than blindly following Privatized heartless American System. There are many religious institutions running hospitals which could be linked with Public Health System.

If i am not wrong, the General Practitioner comes under the definition of the #HealthSysteminIndia - Then he should be made to actively participate on the International Yoga Day, 21st June. If he pays a visit to his regular patients along with a Govt rep and explains them the importance of Yoga in daily life, the citizens will lap up his thoughts easily.

After 67 years of our independence, we are still lagging behind in health services in the country. Health and education shall be treated among the basic needs of people. The ignorance and inability to access health facilities are major problems. People take drugs also without knowing about consequences of it. We cannot go for prescription based drugs only always, but atleast awareness can be made for the harms of self medication. Health centers per 1000 population have to be planned for access

"Swasth sharir-sukhi jeevan". The govt is conducting many schemes on health but due to lazyness of govt and workers, the health services are very bad. In that, it is important to concentrate on following policies- 1. hospital in every village and ward 2. facilitating hospitals 3. good no of doctors and workers 4. no of patient according to doctors(1 per 60) 5. availablity of drugs 6. description till completely healthy 7. cleaness at school level 8. Campeigning of yoga at school level

Connecting the people with health insurance under a national scheme supported by government, employer and self-contribution can work better. There are several countries like Canada and Korea (as I know) have such schemes and running successfully. Honorable PM is coming in Korea, the delegation with him can work out in this direction to make a path on this direction.

इस योजना के लागु होने से कुछ लोगो का आर्थिक नुकसान तो हो सकता हे किन्तु करोड़ो ग्रामीणो का कल्याण होगा एवं 7 लाख आयुष चिकित्सको को लाभ होगा। ग्रामीण क्षेत्रो की स्वास्थ्य सेवाएं बेहतर हो जायेगी एवं देश में चिकित्सको की कमी भी नहीं रहेगी।

इसी सम्बन्ध में 28 /11 /2014 को लोकसभा में प्रश्न क्र ० 949 के उत्तर में केंद्रीय राज्य मंत्री आयुष मंत्रालय श्री श्रीपाद नाईक जी ने कहा हे '' Therefore , allowing practice of Allopathy system by homoeopathic practitioners is within the domain of respective state govt . लोकसभा के प्रश्न क्रमांक 949 से स्पष्ट हो गया हे कि यदि राज्य सरकार चाहे तो बो आयुष चिकित्सको को एलोपैथिक दबाओ की अनुमति दे सकती हे। केंद्र सरकार को , सभी राज्य सरकारों से भी ऐसे कानून बनाने के लिए कहना चाहिए।

ग्रामीण स्वास्थ्य सेबाओ को बेहतर बनाने के लिए आयुष ( आयुर्वेदिक , होम्योपैथिक , यूनानी ) चिकित्सको को निम्नलिखित अधिकार मिलना चाहिए :- 1 . कुछ सामान्य एवं अतिआवश्यक जीवन रक्षक एलोपैथिक दबाओ की अनुमति मिलना चाहिए। 2 .IUCD , प्रसूति , गर्भपात की अनुमति एवं उसमे एलोपैथिक दबाओ के उपयोग की अनुमति। इस योजना के लागु होने से कुछ लोगो का आर्थिक नुकसान तो हो सकता हे किन्तु करोड़ो ग्रामीणो एवं 7 लाख आयुष चिकित्सको को लाभ होगा।

आयुष चिकित्सको को इसकी अनुमति नहीं होने से बो ऐसा नहीं करते , जिससे रोज ग्रामीण क्षेत्रो के लाखो मरीजों को परेसानी होती हे। आयुष चिकित्सको को पूरी एलोपैथिक दबाओ की अनुमति की जरुरत नहीं हे , केबल कुछ सामान्य एवं अतिआवश्यक जीवन रक्षक एलोपैथिक दबाओ की अनुमति की जरुरत हे। सरकार चाहे तो इसके लिए IGNOU से कोई 4 -5 माह के Certificate course का प्रावधान भी किया जाना उचित होगा।
