India has made remarkable achievements in areas like Polio elimination, lowering fertility and disease control. However, our progress in health outcomes has been slower in comparison to other countries with comparable incomes and at similar stages of development. Impressive gains in per capita income should match with increase in life expectancy or health status. We now face a triple burden of disease. Out of pocket expenditures in India is high (70 percent of total health expenditure). This is catastrophic for the poor and pushes an estimated 37 million into poverty every year.

Health is a subject allotted to the State List, under the Seventh Schedule of the Indian Constitution. The Central Government is jointly responsible for items in the Concurrent List.We have one of the most expansive publicly provided networks of health facilities yet issues of regional disparity, access and quality remain. The private sector despite being utilized by the majority of the population also has issues of quality and cost.

Even though the Union Budget allocation for the Ministry of Health and Family Welfare in 2015-16 has remained at the level of revised expenditure in 2014-15, an opportunity lies in encouraging States to spend more on the social sector with greater devolution of untied funds following the recommendations of the Fourteenth Finance Commission.

India is brimming with possibilities. Successful conduct of election, Census survey, projects in space and atomic sciences are some examples. India is termed as the “pharmacy of the global south”, providing affordable, life saving generic medicines to developed and developing countries. In the same way, there is potential for our health system to deliver optimal outcomes to the population.

The Twelfth Plan charts the path towards strengthening health systems so as to reach the long term objective of Universal Health Coverage (UHC). It is our belief that a Health System Strengthening approach is the solution to bridging the gap between our current status and potential performance. The Health Division of the NITI Aayog invites you to an open and informed discussion to elicit ideas for overcoming the enormous challenges in the sector with limited funds at hand and guide future action at all levels, in our system. Your opinion is important and valued.

Detailed note on the current status of health system in India

We invite your responses on these two questions:

1. How can we maximize health returns from available resources?
2. How can we increase investments in health?

This discussion is open for the next two weeks after which we will post a summary of the ideas generated on the forum. We will also follow-up with a more detailed note on the issues in health system and learning from best practices in the country as well as globally for further discussion. Informed briefs on the above mentioned questions will also be made available after the initial two weeks for more a targeted dialogue.

Insurance and Swacch Bharat is going to play a major role in health system in India. Government deserves a big round of applause for this.But please first improve the infrastructure of government hospitals.Many hospitals are left in such an unhygienic condition that even the relatives of the patient are at high risk of getting diseases.Second important basic step is educate the doctors and nurses in government hospitals to at least speak politely to the people coming for a treatment.

There is Heavy Corruption in Private medical practice .It is more than what is happening in Govt dept. Patients are requested to undergo various unnecessary tests. Patient is prescribed heavy dose of medicines to facilitate sell of medicines .Lady is operated for Cesarian though normal delivery could be possible. Dead Patients are kept on ventilator. Angioplasty / kidney removal is done for dead patients. There is unending list of malpractice in private medical practice.

Government may take initiative for helping and guiding common people through telephone. A toll free number to meet the various requirement of common people may provided. Govt should set up a help centre where expert in various social sectors may be available for advising and helping common people. . Sir, please make complusory for every elected candidate (right from gram panchayat to president or primeminister to go only in government hospital in case of any illness/injury/health related problems. This will automaticaly give rise the cleanliness & good service in government hospitals.Today nobody will prefer for government hospital because of the non cleanliness & doughtful services.

t least one govt appointed MBBS doctor should be available in each "Gram Panchayat". Govt can take fees from the public and can run clinic on no profit no Loss base. Appointment of the doctors must be fast as many rural health centers not working because of unavailability of doctors. Each District must have a multiplicity hospital with adequate staff. Build cluster wise special hospitals like cancer hospital were more patients found, i.e. malwa of Punjab and etc..

In india a lot type of treatments are available so the large number of quack doctors are their. Instead of catching and punishing them a system should be developed for catching training identifying their potential and properly issuing licence to them so they can perform better for the well being of nation.

Our focus should change from treatment to prevention. Citizens only go to the hospital when the situation seems out of control. Why health workers should wait in the hospital instead visit schools, offices, localities take care of health issues at the point of occurance. after some one falls sick it take more efforts and time to cure. In india every person knows some kind of treatment. We have to devlope this skill of by introducing ayush as a subject from Class 6 itself.
