India has made remarkable achievements in areas like Polio elimination, lowering fertility and disease control. However, our progress in health outcomes has been slower in comparison to other countries with comparable incomes and at similar stages of development. Impressive gains in per capita income should match with increase in life expectancy or health status. We now face a triple burden of disease. Out of pocket expenditures in India is high (70 percent of total health expenditure). This is catastrophic for the poor and pushes an estimated 37 million into poverty every year.

Health is a subject allotted to the State List, under the Seventh Schedule of the Indian Constitution. The Central Government is jointly responsible for items in the Concurrent List.We have one of the most expansive publicly provided networks of health facilities yet issues of regional disparity, access and quality remain. The private sector despite being utilized by the majority of the population also has issues of quality and cost.

Even though the Union Budget allocation for the Ministry of Health and Family Welfare in 2015-16 has remained at the level of revised expenditure in 2014-15, an opportunity lies in encouraging States to spend more on the social sector with greater devolution of untied funds following the recommendations of the Fourteenth Finance Commission.

India is brimming with possibilities. Successful conduct of election, Census survey, projects in space and atomic sciences are some examples. India is termed as the “pharmacy of the global south”, providing affordable, life saving generic medicines to developed and developing countries. In the same way, there is potential for our health system to deliver optimal outcomes to the population.

The Twelfth Plan charts the path towards strengthening health systems so as to reach the long term objective of Universal Health Coverage (UHC). It is our belief that a Health System Strengthening approach is the solution to bridging the gap between our current status and potential performance. The Health Division of the NITI Aayog invites you to an open and informed discussion to elicit ideas for overcoming the enormous challenges in the sector with limited funds at hand and guide future action at all levels, in our system. Your opinion is important and valued.

Detailed note on the current status of health system in India

We invite your responses on these two questions:

1. How can we maximize health returns from available resources?
2. How can we increase investments in health?

This discussion is open for the next two weeks after which we will post a summary of the ideas generated on the forum. We will also follow-up with a more detailed note on the issues in health system and learning from best practices in the country as well as globally for further discussion. Informed briefs on the above mentioned questions will also be made available after the initial two weeks for more a targeted dialogue.

Respected Prime Minister, Jai Hind !!! Since there is a shortage of MBBS doctors in our country, i suggest that BHMS doctors can be granted an equivalent status by the Medical Council of India subject to certain conditions. For example, the MCI may conduct an exam/orientation course for the certification of BHMS doctors before granting them certificate. This will help in increasing the Medical Work Force which will result in more people getting good quality medical services.

1. Establish primary health centre with blood and x-ray facilities in every village. 2. At block level, hospital with all the specialists with MRI/CT Scan facilities with nominal charges. 3. At district level, super speciality hospitals with high standard. 4. Trainee doctors must be legally bonded to serve in the rural areas for at least 5 years after graduating. 5. The hospitals can be established jointly by three parties i.e. Central Govt, State Govt and PSU/Private companies.

1) Stop doctors from going abroad after studies. 2) Double the internship period of students in hospitals. Half for training and another half for serving people. Pay them for 2nd half of internship as regular salary. 3) Give nurses some more power to give basic medicines to patients for where it doesn't require doctors. Include some Info on basic medicines in their training. 4) Make basic medicines available in all schools. So that teachers can give medicines to children in need for free.

Plz provide a health insurance scheme for all the citizen of india having low premium amount so that everyone can invest on that scheme and have less burden of expence on medical facilities...... sorry for my bad english . :)

Being a medical student i am aware of people's problem. We need a lot to do to make people aware. Cleanliness sanitisation are must. These can reduce infectious diseases by a lot and cut of burden to nation. We need a lots of doctor that too well qualified. Plus health care facilities are to be improved and also payscale of gov docs. Increase it so that they dont turn to private practice. India has still a long way to go in this.

We are thankfull for the service minded doctors for their service. There is rapid commercialisation of medical services,few doctors are charging very high consulting fees earning several lakhs monthly but not even maintaining patient records,health care is fast becoming unaffordable request urgent government intervention to rapidly increase the number of doctors and specialists and to regulate the profession so that they don't overcharge at the cost of patients,let foreign doctors practise here

Pls strictly regulate the medical education and medical profession, by enacting a health regulatory authority to regulate quality of medical education and fees chargable.Doctors who are educated in Govt Colleges should be asked to do compulsory government service or asked to compensate the government.Government to fix consulting fee, hospital charges &take action against doctors who refer patients for unwanted scans/MRI etc.Every patient should be given receipt by the doctor for the fee taken

Thanks for Govt for requesting public views.Contrary to rosy picture of health system painted, the true situation is something else.Approximately 64000 MBBS seats and less than 18000 PG seats on all India basis not at all sufficient for our huge population, result capitation fee in lakhs is paid for MBBS Seats and crores for PG Seats in private sector, the MBBS and PG seats will never increase as then the capitation fee will come down.Pls add PG and MBBS Seats & act against capitation fees
